Traditional Burial Pre-Planning

Here are some items you will want to keep handy and/or pass along to a trusted individual for use at the appropriate time:

  • Date of birth and birthplace
  • Social security number
  • Father’s name and state of birth
  • Mother’s maiden name and state of birth
  • If veteran status applies, Form 214
  • Copy of Driver’s License
